Purpose of the Credit Card Authorization Form
The purpose of this form is for cardholders to authorize other parties to
pick-up material on their behalf without the credit card being present, or, for
cardholders to authorize deliveries to addresses other than the billing
address on the credit card.
This form is only necessary if the cardholder is not present to swipe and
sign for the card at one of our locations.
The authorization form and credit card information is kept confidential in the
Len-Co office.
